Description
Tofacent (Tofacitinib) is used for treating moderate to severe rheumatoid arthritis (RA). This helps reduce pain, tenderness, and swelling in the joints by reducing inflammation. It also slows down the degeneration of bones and joints. Tofacent may be prescribed alone or in combination with other medications. It is wise to take these tablets every day simultaneously, with or without food. The dosage depends on your condition and what other medications you are taking. It would help if you used it according to your doctor’s instructions. Your condition could worsen if you stop taking this medicine without your doctor’s advice.
Infections of the throat and nose, nausea, and viral infections are among the most common side effects of Tofacitinib (Tofacent). You may also develop cold sores, feel sick, or experience symptoms of a cold such as stuffy nose, sneezing, and sore throat. Several less common but serious side effects may occur, including blood clots and tears in the stomach or intestines. Such issues should be treated immediately. What is tofacitinib?
Rheumatoid arthritis is an autoimmune disease characterized by a dysregulation of pro-inflammatory cytokines including IL7, IL15, IL21, IL6, IFN-alpha, and IFN-beta. (3) Cytokines signaling results in tissue inflammation and joint damage by stimulating the recruitment and activation of immune cells via the janus kinase signaling pathway.
The medication tofacitinib is classified as a janus kinase inhibitor (JAK). It works by suppressing the immune system, a disease-modifying anti-rheumatic drug (DMARD). Tofacitinib is available as a tablet (Xeljanz), extended-release tablet (Xeljanz XR), and oral solution. The drug treats certain inflammatory conditions in people who have already tried TNF blockers.
In 2012, the FDA approved tofacitinib as the first JAK inhibitor for the treatment of rheumatoid arthritis and the first new oral DMARD approved for the condition in more than a decade.

Tofacitinib XR 11mg: Cellulose acetate, copovidone, hydroxyethyl cellulose, hydroxypropyl cellulose, HPMC 2910/Hypromellose, magnesium stearate, red iron oxide, sorbitol, titanium dioxide, and triacetin. Printing ink contains ammonium hydroxide, ferrosoferric oxide/black iron, propylene glycol, and shellac glaze.
Tofacitinib 5mg tablets 22mg: Cellulose acetate, copovidone, FD&C Blue #2 Aluminum Lake, hydroxyethyl cellulose, hydroxypropyl cellulose, HPMC 2910/Hypromellose, magnesium stearate, red iron oxide, sorbitol, titanium dioxide, triacetin, and yellow iron oxide. Tofacitinib oral solution: Grape flavor (natural), hydrochloric acid, lactic acid, purified water, sodium benzoate, sucralose, and xylitol.
What is tofacitinib used for?
Rheumatoid Arthritis: First of all, indicate treatment of adult patients with moderately to severely active rheumatoid arthritis who have had an inadequate response or intolerance to methotrexate. This drug is used either alone or in combination with methotrexate or other nonbiologic disease-modifying antirheumatic drugs (DMARDs).
Psoriatic Arthritis: Tofacitinib is indicated to treat adult patients with active psoriatic arthritis who do not respond or do not tolerate methotrexate or other disease-modifying antirheumatic drugs (DMARDs).
Ulcerative Colitis: It indicates the treatment of adult patients with moderately to severely active ulcerative colitis (UC).

If you are allergic to tofacitinib or have any other allergies, tell your doctor or pharmacist. The product may contain inactive ingredients, which can cause allergic reactions. Consult your pharmacist for more information.
Before using this medication, tell your doctor or pharmacist your medical history, especially of: blood clots (such as in the lungs and legs), blood/bone marrow disorders (such as anemia, low lymphocyte/neutrophil count), cancer, heart problems (such as previous heart attack), kidney disease, liver disease (such as hepatitis B or C), lung disease, past/recent/current infections (such as tuberculosis, herpes zoster, HIV infection), current or past smoking, other stomach/intestinal disorders (such as ulcers, diverticulitis), stroke.
Tofacitinib can increase your risk of getting infected or worsen an existing infection. Contact with people who have infections that can spread to others (such as chickenpox, measles, and the flu) should be avoided. For more information or consult your doctor if you have been exposed to an infection.
Before receiving any immunizations or vaccinations, inform your health care provider that you are using tofacitinib. It would be best to avoid contact with people who have recently received live vaccines (such as flu vaccine inhaled through the nose).
Diabetes, liver disease, phenylketonuria (PKU), or any other condition requiring dietary limitations or avoidance is advised. If you have questions about using this product safely, consult your physician.
If you plan on having surgery, tell your doctor about all the medications you are taking (including prescriptions, non-prescription, and herbal products). Using this medication may increase the risk of infection for older adults.
This medication should only be used when necessary during pregnancy. Consult your doctor about the risks and benefits. Let your doctor know if you intend to become pregnant or if you are already pregnant.
It is unknown whether this medication passes into breast milk. The use of this drug while breastfeeding and for at least 18 hours after the last dose is not recommended due to the potential risk to the infant. Before breastfeeding, consult your doctor.
What are the side effects of Tofacent ?
There are serious side effects that may occur when taking this medication. Your doctor prescribed this drug because they deemed that the benefits outweigh the risks. Careful monitoring by your doctor may reduce your risk. Tell your doctor if you experience any serious side effects (unusual fatigue, fast heartbeat, liver disease symptoms).

- Skin or eyes look yellow
- Vomiting
- Fevers
- Stomach discomfort
- Dark urine
Tofacitinib tablets and tofacitinib XR can cause the following side effects in people with rheumatoid arthritis, psoriatic arthritis, and ankylosing spondylitis:
- Infections of the upper respiratory tract (common cold, sinusitis)
- Runny nose, sore throat, and nasal congestion (nasopharyngitis)
- Hypertension (high blood pressure)
- Headache
- Diarrhea
It is rare for this drug to cause a very serious allergic reaction. If you notice any signs of a serious allergic reaction, such as severe dizziness or trouble breathing, get medical help.

Pregnancy and breastfeeding
You should inform your doctor if you plan to become pregnant or are pregnant or if you plan to breastfeed or are breastfeeding. Tofacitinib may interfere with females’ ability to become pregnant. Whether this will change after stopping Tofacitinib is unknown. It is unknown whether Tofacent will harm an unborn baby.

Storage Conditions
Keep away from light and moisture at room temperature. Avoid storing it in the bathroom. Keep children and pets away from all medications.
Be sure to store the liquid form tightly closed in the original container. Place the bottle in the carton after each use to protect it from light. After 60 days, dispose of any remaining liquid medication.
